Interestingly enough, I finally moved to Vista myself over the Christmas break. It was definitely a learning experience…especially with the issue you are describing. I figured out that the OINSIGHT.INI file is no longer stored in the <root>:\Windows folder anymore. However, your old one might still be there if all you did was do an upgrade. This file is now stored in <root>:\Users\<user name>\AppData\Local\VirtualStore\Windows.
If you can’t find your original file, here is what my snippet of OINSIGHT.INI looks like:
SideBarCustom_Name=SRP Editor,SRP Table Search
SideBarCustom_Text= Launch the SRP Editor,Launch the SRP Table Search Utility
SideBarCustom_Img = C:\Windows\SRPEditorIcon2.bmp, C:\Windows\SRPTableSearchIcon.bmp
SideBarCustom_Cmd = OIWINEXE**SRP_EDITOR, OIWINEXE**SRP_TABLE_SEARCH
